Minnesota assisted living & 245D group home software

Run your 144G assisted living and 245D group homes on one platform built for the Minnesota survey — resident records, service plans, ISP notes, eMAR, incident reporting and, where your services require it, EVV.

New to Minnesota compliance? Start with the Minnesota overview — 245D, 144G, CFSS and EVV in one place.

$1

Does EVV apply to assisted living?

Not to 144G assisted living itself. DHS's published list of services that require EVV covers personal care and CFSS, home health, and certain waiver and 245D services such as individualized home supports. 144G assisted living is not on it, though DHS notes the list may expand.

If you also deliver EVV-covered services, those visits are held to Minnesota's threshold: at least 80% compliance on all visits billed since July 1, 2026, up from 50% on January 1. Formal corrective action notices began in April 2026 for providers below 50%, and notices to providers below 80% begin in October 2026.

Not sure which of your services are covered? See the EVV guide and check your service codes against the DHS list.

Two licenses, two regulators, one set of residents

A lot of Minnesota operators hold a 144G assisted living license from MDH and a 245D HCBS license from DHS, sometimes for people living in the same building. The regulators are different, the documentation expectations are different, and the surveys are separate.

What that produces in practice is two parallel record systems — a 144G service plan and contract on one side, a 245D individual service plan with progress notes and outcomes on the other — kept by the same overworked staff. Things drift out of sync, and the drift is what gets found.

Chapter 144G was passed in 2019 and has been amended repeatedly since, including changes in the 2025 session and a 2026 change affecting new MDH-licensed facilities with a licensed resident capacity of six or fewer. If you are a small operator, the rules that apply to you have moved recently.

Built for how Minnesota actually surveys

Resident & tenant records

Photo, diagnoses, service plan and emergency contacts for every resident across all your homes — one record, not a binder per building.

144G service plans & contracts

Assisted living service plans and resident contracts documented in the system that holds the care record, so they don't drift apart.

245D ISP & progress notes

Documentation written against the Individual Service Plan with structured progress notes and outcomes tracking for waivered services.

eMAR with audit trail

Every medication pass timestamped and attributed, misses flagged when they happen — defensible in an MDH or DHS survey.

Incident reporting & MAARC

Structured incident and behavior reporting aligned to DHS and MAARC expectations, captured at the time of the event with notifications logged alongside.

EVV for covered 245D services

For services that require EVV, GPS visit verification submitted to HHAeXchange with a running compliance percentage, so you see a shortfall forming instead of receiving a notice about it.

Staff training & credentials

Orientation, competency and annual training tracked per person with expiry alerts across every home.

Survey-readiness export

One-click export of the documentation set an MDH or DHS licensor typically asks for, rather than three days of assembling paper.

The 24-hour reporting clock

Certain incidents — serious injuries, deaths, medical emergencies, mental health crises, law enforcement involvement, unauthorised absences — must be reported within 24 hours to legal representatives, case managers and DHS Licensing. Behavioral interventions require the Behavioral Intervention Report Form (DHS-5148).

Twenty-four hours is unforgiving when the initial account lives in a staff member's memory and gets written up two shifts later. Capturing it on a phone when it happens, with notifications logged, is the difference between a defensible file and a reconstruction.

$199 a month, flat, across up to five resident homes

$199 per month covering up to five resident homes with unlimited employees. No per-resident fee, no per-seat fee, no per-DSP fee, month-to-month.

Per-seat pricing is particularly punishing in residential services, where DSP turnover is high and you end up paying for accounts belonging to people who left. That math never worked for Minnesota operators, which is why we don't use it.

Implementation is about a week to live and 30 days to fully transition.

Common questions

Does this cover both 144G assisted living and 245D group homes?

Yes, on one login. Many Minnesota operators hold both licenses and end up running two systems or two binders. Sothcare handles 144G service plans and contracts alongside 245D ISP documentation, progress notes and outcomes.

Does Minnesota EVV apply to assisted living?

Not to 144G assisted living itself. DHS's published list of services that require EVV covers personal care and CFSS, home health, and certain waiver and 245D services such as individualized home supports, and DHS notes the list may expand. If you deliver EVV-covered services, the threshold is 80% of visits billed since July 1, 2026, and DHS notices to providers below 80% begin in October 2026.

Who regulates which part?

MDH licenses assisted living facilities under Minnesota Statutes Chapter 144G. DHS licenses 245D home and community-based services. Operators holding both answer to both, which is exactly why running them in one system matters.

Does it handle incident and maltreatment reporting?

Yes — structured incident and behavior reporting aligned to DHS and MAARC expectations, with a survey-readiness export. Certain incidents must be reported within 24 hours to legal representatives, case managers and DHS Licensing, and behavioral interventions require form DHS-5148.

How much does it cost?

$199 per month, flat, covering up to five resident homes with unlimited employees. No per-resident fee, no per-seat fee, no per-DSP fee, month-to-month.
