The honest version
Axxess is a serious product with a serious following. Its training and certification ecosystem is genuinely good, its mobile apps are well built, and a lot of agencies are happy on it. Anyone telling you Axxess is bad software is selling something.
The question is fit. Axxess is built to scale an agency across states and service lines. Sothcare is built for an independent agency that wants clinical documentation, EVV and billing on one bill that does not move when headcount does.
Those are different problems, and the pricing models reflect it.
Figures are from public software directories, not from Axxess. Axxess does not publish pricing, and actual cost varies by contract, module selection and agency size. Verify directly before deciding on cost. Last checked August 2026. Run your own numbers →

Where the per-user model bites hardest
Skilled home health runs on a clinician roster that changes. Under per-user pricing that produces two costs: you keep paying for accounts belonging to clinicians who left until someone deprovisions them, and you pay again when you hire the replacement.
It also produces a quieter cost. When a login has a price attached, small agencies ration them, and shared accounts appear. At that point the audit trail stops being attributable to an individual — which is the entire reason a surveyor wants an electronic record in the first place.
A flat fee removes both. Every clinician gets their own login because it costs nothing extra, and your invoice in a hiring month matches your invoice in a quiet one.
Where Axxess is genuinely the better choice
Hospice and palliative lines. If you run those today, or expect to within a year, Axxess covers them and we do not.
Scaling across many states. Axxess is built for multi-state operations. We are deepest in Minnesota and expanding state by state.
Training and certification. If you onboard clinicians in volume and want a structured curriculum with certificates, that ecosystem is a real asset and we have nothing equivalent.
If any of those describe you, we would rather say so now than sell you something you leave in four months. See who Sothcare is built for →
Where we think we win for an independent agency
Cost at small scale. $499 flat for skilled home health at any headcount. Run it against your own invoice rather than our estimate.
PDGM at the point of care. The CMS HHGS grouper runs live in the home, so the HIPPS code is visible before the clinician leaves.
Minnesota depth. If you also hold a 245D or 144G license, the residential and assisted living work sits on the same login rather than in a second system.
Who answers support. A founder, at our current size. That stops being true above some scale, and we would rather say so than pretend otherwise.

Is Sothcare cheaper than Axxess?
For most independent agencies, yes. Axxess is census-tiered with per-user charges; Sothcare is a flat $499/mo for skilled home health regardless of size.
Can I migrate without losing claims?
Yes. Active episodes finish in Axxess while new admissions start in Sothcare; data imports via CSV. No claims lost.
Questions to ask before you switch from Axxess
Whichever vendor you choose, get these answers in writing:
- What is my total monthly cost at my current clinician count, and at 25% more clinicians, including add-on modules?
- Are there per-episode, per-claim or per-visit charges on top of the base price?
- What does implementation cost, and what does it include?
- What is the contract term, and what does it cost to leave?
- How do I export my data if I leave, and in what format?
- How are OASIS-E2 and PDGM handled at the point of care?
- Who do I call for support, and how fast do they respond?
What switching to Sothcare involves
- You run Sothcare in parallel with your existing system. You do not cut over until claims are landing correctly on ours.
- Skilled Home Health: about 30 days to a first parallel episode and 60–90 days to full cutover. We handle the patient and clinician import.
- $499 per month, flat, with unlimited staff accounts. Month-to-month, with no annual contract.
- A one-time implementation fee applies and varies by service line. We quote it before you commit, and sandbox provisioning and data import are included in it.
